Introduction: Hospitals are among the most important physical infrastructures of the health system and allocate a significant share of capital resources. The quality of design, construction, and operation of hospital buildings has a direct impact on productivity and quality of services. However, due to the complexity of health services and the absence of comprehensive and localized frameworks, the productivity assessment of hospital buildings has been less systematically examined. This study was conducted with the aim of developing a framework for assessing the productivity of hospital buildings in Iran.
Methods: This study was conducted using the scoping review method based on the six-stage framework of Arksey and O’Malley. Systematic search without time limitation was conducted in PubMed, ScienceDirect, Web of Science, Springer, SID, Magiran, and Google Scholar from April to November 2025. After screening titles, abstracts, and full texts, data were extracted and analyzed using the building life-cycle approach.
Results: Among 530 identified sources, 54 studies were included in the final analysis. The results showed that hospital building productivity can be classified into three stages: pre-construction, construction, and post-construction. The main components of these stages included “compliance with upstream documents,” “quality of hospital construction,” and “building operation.” In addition, indicators related to planning, technical quality, safety, energy management, space efficiency, and maintenance were identified.
Conclusion: Hospital building productivity is a multidimensional concept that should be assessed throughout the entire building life cycle. The proposed framework can serve as a basis for designing localized productivity assessment tools and supporting managerial decision-making and policymaking in the health system.
