fa عدالت در مشارکت مالی خانوار در نظام سلامت در استان آذربایجان شرقی Fairness in household financial contribution to the health system in East Azerbaijan province عمومى General مروری Review مقدمه: عدالت در مشارکت مالی خانوارها یکی از اهداف مهم نظام سلامت است. بنابراین هدف از این مطالعه محاسبه ی میزان مواجهه ی خانوارها با هزینه‌ی کمرشکن، و میزان عدالت درمشارکت مالی خانوارهای استان آذربایجان شرقی در هزینه های سلامت و عوامل موثر برآن است. روش کار: این مطالعه با استفاده از داده های ثانویه ی حساب های سلامت استان آذربایجان شرقی در سال 1397 به محاسبه میزان پرداخت از جیب و هزینه های کمرشکن خانوارها و میزان عدالت در مشارکت مالی در هزینه های سلامت می پردازد. نمونه مـورد بررسی ، شامل  1721 خانوار بود. از شاخص جینی(Gini coefficient) برای سنجش توزیع درآمد، از شاخص تمرکز برای توزیع هزینه های سلامت و از شاخص کاکوانی Kakwani index) ) برای تخمین عدالت در مشارکت مالی خانوار استفاده شد. یافته ها: میانگین پرداخت از جیب خانوارها  برابر با  12،118،098.18 با انحراف معیار 4566976 ریال است که در خانوارهای پنجک بالاتر بیشتر از پنجک پایین است.  13.53 درصد دچار هزینه ی کمرشکن شده‌اند. میزان شاخص جینی برای توزیع درآمد برابر  0.25 ، شاخص تمرکز برای توزیع هزینه های سلامت برابر 0.36 و شاخص کاکوانی برای عدالت در مشارکت مالی برابر 0.11 بدست آمد. نتیجه گیری:  توزیع هزینه های سلامت نسبت به درآمد به نفع  فقرا است و عواملی مثل داشتن بیمه و وجود افراد مسن در خانوار و تحصیلات و سن سرپرست خانوار رابطه ی معنی داری با میزان مشارکت در تأمین مالی هزینه های سلامت دارند.  بنابراین توصیه می شود پوشش و عمق بیمه پایه گسترش یابد. Introduction: Equity in household financial participation is one of the important goals of the health system. The purpose of this study is to calculate the extent of household’s exposer to <a name="_Hlk96509699">catastrophic expenditures</a>, and the degree of justice and participation in the financing of households and the impact of factors affecting participation in financing. Methods: This descriptive and analytical study, which uses the secondary data of health accounts of East Azerbaijan province in 2018 to calculate the amount of out-of-pocket and household <a name="_Hlk96509816">catastrophic </a>expenditures. The sample of this study included 1721 households. The distribution of household income using the Gini coefficient and the distribution of health expenditures with index concentration and household fairness in financial contribution using the Kakwani index. Result: The average out-of-pocket payment for households is 12,118,098.18 and the standard deviation is 4566976, which is higher in households in the top quintile than in the bottom quintile. 13.53% have incurred <a name="_Hlk100904578">catastrophic expenditure</a>. The Gini index was 0.25 the concentration index was 0.36 and the Kakwani index was 0.11. Conclusion: Distributing health expenditures relative to income benefits the poor. And the impact of factors such as insurance and the presence of aged people in the household and education and age of the head of the household have a significant relationship with the level of participation in financing health costs.
