fa بررسی تغییرات در سهم منابع تامین مالی هزینه های صورتحساب بیماران در زمان کووید-19 نسبت به قبل از آن Investigating the changes in the share of financing sources of patient billing costs during the time of Covid-19 compared to before عمومى General پژوهشی Original مقدمه: یکی از  مباحث جذاب  و  درعین‌حال چالش‌برانگیز در نظام های سلامت، نحوه تامین منابع مالی هزینه های درمان بیماران است. در شرایط پاندمی کووید-19 این امکان وجود داردکه سهم منابع تامین مالی هزینه های درمان بیماران دچار تغییراتی شود. مطالعه حاضر با هدف بررسی تغییرات در سهم منابع تامین مالی هزینه های درمان بیماران در دوره قبل و حین کووید-19 انجام شد. روش کار: این مطالعه توصیفی-تحلیلی با رویکرد کاربردی انجام شد. محیط پژوهش دو بیمارستان مرجع کووید-19 در دانشگاه علوم پزشکی شهر مشهد بود. 305382 پرونده موردبررسی قرار گرفت. منبع اصلی برای جمع‌آوری داده ها سیستم اطلاعات بیمارستانی بود. تجزیه‌ و تحلیل داده ها در نرم‌افزار EXCEL 2017 و SPSS نسخه 25 با استفاده از آزمون من ویتنی در سطح معناداری 0/05 انجام گرفت. یافته‌ها: یافته ها نشان داد سهم بیمه پایه در دوره قبل نسبت به دوره حین کووید-19 به ترتیب برابر با حدود 70 و 72 درصد بوده است (0/090=P). نسبت سهم یارانه سلامت از 4.8 در دوره قبل به 2.2 در دوره حین کووید-19رسیده است (0/001>P). مقایسه نسبت سهم پرداختی بیمار به مبلغ کل پرونده برای بیماران داخلی-عفونی در دوره قبل و بیماران مبتلابه کووید-19 در دوره حین کووید - 19 نشان‌دهنده افزایش حدود 7 درصدی است (0/001>P). نتیجه‌گیری: نتایج حاصل از مطالعه حاکی از آن است که همزمان با شیوع پاندمی کرونا سهم هر یک از منابع تامین مالی صورتحساب بیماران دستخوش تغییراتی شده است. افزایش سهم بیماران مبتلابه کووید-19نشان دهنده نیاز به برنامه هایی مدون برای حفاظت مالی از گیرندگان خدمات سلامت در زمان مواجهه با بحران های احتمالی در آینده است. Introduction: One of the interesting and challenging topics in healthcare systems is the methods of financing the costs of patient treatment. During the COVID-19 pandemic, the share of financial resources for patient treatment may have changed. This study was conducted to examine the changes in the share of financial resources for patients’ hospital bills during the pre-COVID-19 and COVID-19 periods. Methods: This descriptive-analytical study was conducted with an practical approach. The research environment was two the COVID-19 referral hospitals at Mashhad University of Medical Sciences. A total of 305,382 files were reviewed. The main source for data collection was the hospital information system. Data analysis was performed using Excel 2017 and SPSS version 25 software, using the Mann-Whitney test at a significance level of 0.05. Results: The results showed that the share of basic insurance in the pre-COVID-19 period and in the covid-19 period was 70% and 72%, respectively(P=0.09). The ratio of health subsidy share decreased from 4.8 in the pre-COVID-19 period to 2.2 in the COVID-19 period (P<0.001). The ratio of patient payment share compared to the total files amount for internal-infectious patients increased by about 7% in the COVID-19 period compared to the pre-COVID-19 period (P<0.001). Conclusion: The results of the study indicate that with the spread of the COVID-19 pandemic, the share of each financial resource in the patient’s bill has changed.
