fa طراحی مدل برنامه ثبت ملی هیسترکتومی برای ایران Designing a National Hysterectomy Registry Model for Iran عمومى General پژوهشی Original مقدمه: هیسترکتومی یکی از رایج‌ترین عمل‌های جراحی در سراسر جهان است که به دلیل بیماری‌های خوش‌خیم، سرطان‌های بدخیم، موارد اورژانس پس از زایمان و تغییر جنسیت انجام می‌شود. علاوه بر عوارض احتمالی جراحی، خروج رحم پیامد‌هایی متعددی به دنبال دارد. طراحی و ایجاد یک برنامه ثبت ملی با اطلاعات یکپارچه می‌تواند در ارزیابی و اثر بخشی هیسترکتومی، برنامه‌ریزی برای ارتقای کیفیت مراقبت و تحقیقات موثر باشد. روش کار: این پژوهش توصیفی در سال1401-1402 انجام شد. بر اساس بررسی متون (جستجو در پایگاه‌های PubMed، Scopus، Web Of ScienceوCochrane Library) و مطالعه تطبیقی برنامه ثبت هیسترکتومی در کشورهای پیشگام، پرسشنامه ای تدوین شد و پس از تایید پایایی و روایی در اختیار 40 نفر از خبرگان قرار گرفت. پس از طی دو مرحله ( تکنیک دلفی)، مدل نهایی برنامه ثبت ملی هیسترکتومی برای ایران ارائه شد. یافته‌ها: مدل برنامه ثبت ملی هیسترکتومی شامل پنج بخش اصلی است که عبارتند از: اهداف، ساختار، مجموعه حداقل داده‌ها (داده‌های اداری- بالینی، داده‌های مربوط به هیسترکتومی، مرگ بیمار و داده‌های بیمارستانی)، استاندارد‌ها و فرآیند‌ها (پردازش‌ها، گزارشات، کنترل کیفی و پیگیری) که مورد توافق اکثریت خبرگان قرار گرفت. نتیجه‌گیری: مراحل پیشهاد شده در مطالعه حاضر می‌تواند رویکردی برای طراحی سایر برنامه‌های ثبت باشد. همچنین، با توجه به نقش محوری برنامه ثبت هیسترکتومی در مدیریت خدمات بهداشتی و درمانی، اجرای منظم و پیوسته این برنامه در تمامی مراکز زنان و زایمان پیشنهاد می‌شود. Introduction: Hysterectomy is one of the most common surgical operations worldwide, which is performed due to benign diseases, malignant cancers, postpartum emergencies, and gender reassignment. In addition to the possible complications of surgery, the removal of the uterus has many consequences. Designing and creating a national registry program with integrated information can be effective in the evaluation and effectiveness of hysterectomy, planning to improve the quality of care and research. Methods: This descriptive research was conducted in 2022-2023. After reviewing the texts from PubMed, Scopus, Web of Science and Cochrane Library databases and based on the comparative study of the hysterectomy registration program in pioneer countries, a Delphi questionnaire was developed and after confirming the reliability and validity, it was given to 40 experts. After two stages, the final model of the national hysterectomy registration program for Iran was presented. Results: The National Hysterectomy Registry Program model includes five main categories: objectives, structure, minimum data sets (administrative-clinical data, data related to hysterectomy and patient death, and hospital data), standards and processes (processing, reports, quality control and follow-up), which was agreed by the majority of experts. Conclusion: The steps performed in this study could be an approach to designing other registration programs.
