fa شناسایی چالش‌های موجود در نحوه‌ی استقرار سامانه‌ی یکپارچه‌ی بهداشت (سیب) و اجرای آن از دیدگاه کاربران محیطی و ستادی دانشگاه علوم پزشکی تهران یک سال پس از اجرا (سال 1396): یک مطالعه‌ی کیفی عمومى General پژوهشی Original <div style="text-align: justify;"><span style="font-size:12px;"><span style="font-family:Tahoma;"><strong>مقدمه<span dir="LTR">‌</span></strong>: شناسایی چالش‌ها و موانع موجود در استقرار و اجرای موفق سامانه‌ی یکپارچه‌ی بهداشت از دید کاربران محیطی و ستادی می‌تواند اطلاعات ارزشمندی را برای مجریان و سیاست‌گذاران سطوح استانی و ملی فراهم نماید.<br> <strong>روش کار<span dir="LTR">‌</span></strong>: تحقیق حاضر، مطالعه‌ای کیفی بود. داده‌ها از طریق مصاحبه‌ی نیمه‌ساختاریافته تا رسیدن به مرحله‌ی اشباع اطلاعات ادامه‌یافت. نمونه‌گیری به‌صورت هدفمند در سال 1397 از میان اعضای تیم ارایه‌دهندگان خدمت، کارشناسان و مدیران ستادی مرکز بهداشت جنوب تهران و معاونت بهداشت دانشگاه علوم پزشکی تهران انجام شد. تحلیل داده‌ها به‌روش تحلیل محتوای کیفی متعارف و با استفاده از نرم‌افزار <span dir="LTR">MAXQDA</span> انجام گرفت.<br> <strong>یافته<span dir="LTR">‌</span>ها<span dir="LTR">‌</span></strong>: تجزیه‌و‌تحلیل داده‌های مصاحبه منجر به ظهور 504 کد اولیه و هشت طبقه گردید. مضامین حاصل‌شده شامل تعریف سامانه، نظر شرکت‌کنندگان در‌خصوص سامانه، مزایای طرح، معایب طرح‌، اجرای سراسری در کشور، چالش‌های اجرای موفق برنامه، شرایط اجرای طرح و پیشنهاد برای رفع مشکلات موجود بود.<br> <strong>نتیجه<span dir="LTR">‌</span>گیری<span dir="LTR">‌</span></strong>: نتایج مطالعه‌ی انجام‌شده نشان داد که سامانه‌ی یکپارچه‌ی ثبت پرونده‌های الکترونیک سلامت، برای ارتقای نظام سلامت کشور ضروری است، اما مشکلاتی پیرامون راه‌اندازی و اجرا و همچنین برنامه‌نویسی سامانه وجود دارد که رفع آن‌ها نیازمند هماهنگی و همکاری در سطوح مختلف اجرایی است. همچنین، شرکت‌کنندگان تجربه‌ی دانشگاه علوم پزشکی تهران را در صورت رفع مشکلات و نواقص سامانه، قابل‌تعمیم به کل کشور دانستند.<br> <strong><span dir="LTR" style="letter-spacing:.3pt;"><span style="color:black;"><span style="font-family:B Lotus;"></span></span></span></strong></span></span></div> <div style="text-align: justify;"><strong>Introduction</strong>: Identifying the challenges and barriers to the successful deployment and implementation of an integrated health care system from the perspective of the field and residential user staff can provide valuable information for administrative staff and policymakers at the provincial and national levels.<br> <strong>Methods:</strong> The present study was qualitative research. Data were obtained through semi-structured interviews, which continued until data saturation. Purposeful sampling was performed in 2018 among service providers, experts, and managerial staff from the South Tehran Health Center and Tehran University of Medical Sciences. Data were analyzed using standard qualitative content analysis with MAXQDA software.<br> <strong>Results: </strong>The analysis of the interview data resulted in the emergence of 504 initial codes and eight themes. The themes included 1) System definition, 2) Comments on the system, 3) Project benefits, 4) Project disadvantages, 5) National implementation, 6) Challenges of successful implementation of the project, 7) Project implementation conditions, and 8) Suggestions to resolve the existing problems.<br> <strong>Conclusion:</strong> The integrated electronic health record system is essential for upgrading the country&rsquo;s health system. But there are problems in setting up, running, and programming the system, which requires coordination and collaboration at different levels of management.
