fa بررسی فراوانی بهره‌مندی از خدمات دندان‌پزشکی به‌دنبال اجرای طرح تحول نظام سلامت در استان اصفهان Dental Care Utilization After the Implementation of “Health Reform Plan” in Isfahan Province عمومى General پژوهشی Original <div style="text-align: justify;">مقدمه‌: طرح تحول نظام سلامت در دندان&lrm;پزشکی، از سال ۱۳۹۳ به صورت سازمان یافته در روستاها و شهرهای با جمعیت کمتر از بیست&lrm;هزار نفر اجرا می&lrm;شود. هدف از مطالعه حاضر، بررسی تاثیر این طرح در میزان بهره&lrm;مندی از خدمات دندان&lrm;پزشکی در استان اصفهان بود. روش کار‌: ده مرکز با دندان&lrm;پزشکانی در قرارداد با طرح تحول انتخاب شدند. خدمات مورد بررسی معاینات، خدمات پیش&lrm;گیرانه، خدمات ترمیمی و کشیدن دندان بود. چهار مرکز که خارج از طرح تحول بودند، به&lrm;عنوان گروه کنترل در نظر گرفته شدند. جهت مقایسه فراوانی خدمات، از آزمون ناپارامتریک من&lrm;ویتنی و ویلکاکسون بهره برده شد (نرم&lrm;افزار SPSS 22 و =α۰۵/۰. یافته‌ها‌: در مراکز کنترل به صورت تجمیعی فراوانی خدمات سال 1393 و 1395 اختلاف معنی&lrm;داری نشان نداد. در مراکز مداخله گروه سنی زیر ۶ سال معاینه (۰۲/p=0)، ترمیم آمالگام (۰۴/p=0) کامپوزیت (۰۴/p=0) و در گروه سنی 12-6 سال معاینه (۰۰۷/p=0)، کشیدن (۰۰۷/p= 0)، ترمیم آمالگام (۰۰۷/p= 0)، کامپوزیت (۰۰۷/p= 0) و جرم&lrm;گیری (۰۱۲/p= 0) و در زنان باردار کشیدن (۰۱۷/p= 0)، ترمیم کامپوزیت (۰۴/p= 0) و جرم&lrm;گیری (۰۱۲/p= 0) افزایش معنی&lrm;دار داشتند. تفاوت میانگین خدمات کودکان زیر ۶ سال، در کشیدن (۰۳۶/p= 0) و ترمیم آمالگام (۰۲۴/p= 0) و در گروه سنی 12-6 سال در کشیدن (۰۱۴/p= 0) و ترمیم آمالگام (۰۰۸/p= 0) به طور معنی&lrm;دار افزایش داشته است. نتیجه‌گیری‌: بعد از طرح تحول دندان&lrm;پزشکان راغب به ارائه خدمات پیشگیرانه (فلورایدتراپی، فیشور سیلانت و جرم&lrm;گیری) و درمانی (کشیدن دندان، ترمیم آمالگام و کامپوزیت) شده&lrm;اند که علت آن را می&lrm;توان به اصلاح روش پرداخت نسبت داد. </div> <div style="text-align: justify;">Introduction: Health reform plan in dentistry has been implemented systematically in Iran since 2014 in villages and cities with a population lower than 20,000 inhabitants. This study aimed to evaluate the impact of this plan on the utilization of dental services in Isfahan province. Methods: Ten health centers with dentists contracted with the health reform plan were recruited. The frequency of dental services (examination, preventive care, restorative care, and extraction) was assessed. Four centers not in contract with this plan were considered as the control centers. To compare the frequency of provided services, the non-parametric tests of Mann-Whitney and Wilcoxon (α=0.05) were used (SPSS 22). Results: In the control centers, the cumulative frequency of provided care demonstrated no significant difference between 2014 and 2017. In the centers subject to the reform plan, there were increases in examination (p=0.02), amalgam (p=0.04), and composite restoration (p=0.04) in the age group of 6 years, and examination, extraction, amalgam, and composite restoration (p=0.007) in the age group of 6-12 years. In pregnant women, extraction (p=0.017), composite restoration (p=0.04), and scaling (p=0.012) increased significantly. In the six-year-old group, the means difference of extraction (p=0.036) and amalgam restoration services (p=0.024) and in the age group of 6-12 years, the means of extraction (p=0.014) and amalgam restoration (p=0.008) were significantly increased in the centers subject to the reform plan than in the control group. Conclusion: After the implementation of the reform plan in health centers, dentists were more willing to provide preventive and restorative care that could be partially due to financial incentives.  </div> <table border="0" cellpadding="0" cellspacing="0"> <tbody> <tr> <td style="width: 720px; height: 72px; text-align: justify;">Please cite this article as follows: Tahani B, Rezayatmand R, Rahimzamani A.
