fa برآورد هزینه‌های درمانی فناوری تزریق سلول‌های تک‌هسته‌ای مشتق از مغز استخوان خود بیمار مبتلا به نارسایی قلبی در مقایسه با روش‌های درمانی رایج در ایران عمومى General پژوهشی Original <div style="text-align: center;"></div> <div style="text-align: justify;"><span style="color:#0000FF;"><span style="font-family: b roya;"><span style="font-size: 12pt;"><strong><span style="font-size: 14px;">مقدمه:</span></strong></span></span></span><span style="color:black;"><span style="font-size:14px;"><span style="font-family: b lotus;"> نارسایی قلبی یک بیماری مزمن است که جزو مه‌مترین علل مرگ‌و‌میر در دنیا می‌باشد و این امر، سالانه هزینه‌های زیادی را به جامعه وارد می‌سازد. هدف این مطالعه برآورد هزینه‌های مستقیم درمانی فناوری تزریق سلول‌های بنیادی مشتق‌شده از مغز استخوان خود بیمار در مقایسه با سایر روش‌های جایگزین در ایران می‌باشد.</span></span></span><br> <span style="font-size:14px;"><span style="color:#0000FF;"><span style="font-family: b roya;">روش کار:</span></span><span style="color: black;"><span style="font-family: b lotus;"> به منظور برآورد هزینه‌های درمان با فناوری تزریق سلول‌های تک‌هسته‌ای مشتق از مغز استخوان(</span></span><span dir="LTR"><span style="color: black;"><span style="font-family: times new roman,serif;">BMC</span></span></span><span style="color: black;"><span style="font-family: b lotus;">) در مقایسه با روش‌های درمانی جایگزین تنها هزینه‌های مستقیم درمانی در نظر گرفته شده است. دیدگاه مطالعه وزارت بهداشت و ارایه‌دهنده خدمت می‌باشد. هزینه‌ها بر حسب تعرفه‌های دولتی و خصوصی در سال 1395 و با استفاده از نرم‌افزار </span></span><span dir="LTR"><span style="color: black;"><span style="font-family: times new roman,serif;">Excel</span></span></span><span style="color: black;"><span style="font-family: b lotus;"> محاسبه و تحلیل گردید.</span></span><br> <span style="color:#0000FF;"><span style="font-family: b roya;"><strong>یافته</strong></span><strong><span style="font-family: b lotus;">‌</span><span style="font-family: b roya;">ها:</span></strong></span><span style="color: black;"><span style="font-family: b lotus;"> هزینه‌های درمانی به ازای هر شخص طی دوره‌ی درمانی به ترتیب بر حسب تعرفه‌های بخش دولتی و خصوصی در سال 1395، به وسیله‌ی سلول‌های بنیادی برای یک دوره‌ی درمانی یک ساله برابر 120 و 222، برای دفیبریلاتورهای قلبی برابر 122 و 218، برای کارگذاری ضربان سازها برابر 120 و 214، برای دارو درمانی برابر 68 و 140 و برای بالن پمپ طی یک دوره‌ی درمانی 10 روزه برابر با 138 و 233 میلیون ریال می‌باشد. به طوری که هزینه‌های درمانی بر اساس تعرفه‌های بخش خصوصی تقریباً بین 7/1 تا 2 برابر هزینه‌های درمانی بخش دولتی می‌باشد. همچنین در هر دو بخش دارودرمانی دارای کمترین هزینه و بالن پمپ به دلیل هزینه‌های بالای بستری، دارای بیش‌ترین هزینه می‌باشد.</span></span><br> <span style="color:#0000FF;"><span style="font-family: b roya;"><strong>نتیجه</strong></span><strong><span style="font-family: b lotus;">‌</span><span style="font-family: b roya;">گیری:</span></strong><span style="font-family: b lotus;"><strong> </strong></span></span><span style="color: black;"><span style="font-family: b lotus;">در کل نتایج نشان داد که از منظر هزینه‌ای فناوری سلول‌های بنیادی با سایر روش‌های درمانی تفاوت چشمگیری ندارد و تقریباً هزینه مشابهی دارند. لذا، پیشنهاد می‌شود که برای ورود این فناوری به داخل کشور به جنبه اثربخشی آن نیز توجه شود و مطالعه هزینه اثربخشی انجام شود. </span></span></span></div> <div style="text-align: center;"><span style="color:#0000FF;"><strong>An Estimation of Costs of Intracoronary/Intramyocardial Injection Technology of Patient Bone Marrow Cell (BMC) Affiliated to Heart Failure in Comparison with Common Treatment Methods in Iran</strong></span></div> <div style="text-align: justify;"><br> <strong>Mojtaba Jafari (BSc)¹; Farhad Lotfi (PhD)¹; Javad Kojuri (MD)²; Hossein Behnam Morshedi (BSc)¹, Khosro Keshavarz (PhD)^1*</strong></div> <div style="text-align: justify;"></div> <div style="text-align: center;"><em>¹Health Human Resources Research Center, Department of Health Economics, School of Management and<br> Information Sciences, Shiraz University of Medical Sciences, Shiraz, Iran<br> ²Quality Improvement in Clinical Education Research Center,<br> Shiraz University of Medical Sciences, Shiraz, Iran</em></div> <div style="text-align: center;"> <div style="text-align: justify;"><span style="color:#0000FF;"><strong>Abstract</strong></span><br> <br> <strong><span style="color:#0000FF;">Introduction:</span></strong> Heart failure is a chronic disease that is one of the most important causes in the world. This subject puts large costs on society. The object of this study is the estimation of direct medical cost on injection technology of patient bone marrow cell affiliated to heart failure in comparison with common treatment methods in Iran.<br> <br> <strong><span style="color:#0000FF;">Methods:</span></strong> In order to estimate treatment cost of injection technology of patient bone marrow cell affiliated to heart failure in comparison with common treatment methods, only direct medical costs were considered. The study perspective was ministry of health and providers. Costs were calculated with Excel software based on private and public tariff.<br> <br> <strong><span style="color:#0000FF;">Results: </span></strong>In 2016, treatment cost per capita based on public and private tariff were respectively 120 and 222 million IR Rial for bone morrow cell, 122 and 218 IR Rial for CRT, 120 and 214 for peace maker, 68 and 140 for pharmacotherapy in a one-year period, and also 138 and 233 million IR Rial for IABP in a 10 day period. Therefore, the treatment cost based on private tariff is almost among 1.7 to 2 time&rsquo;s public tariff. In addition, in terms of cost, pharmacotherapy had the least and IABP had the most.<br> <br> <strong><span style="color:#0000FF;">Conclusions:</span></strong> In general, the result showed that the cost of bone marrow cell had no significant difference with other treatment methods and the costs were almost similar.
