fa کسورات بیمه‌ای بیمارستان‌های ایران: مرور نظام مند علل و راهکارهای کاهش کسورات عمومى General مروری Review <strong><span style="color:#0000FF;"><span style="font-family:b roya;"><span style="font-size:12.0pt;">مقدمه</span></span></span><span style="color:black;"><span style="font-family:b roya;"><span style="font-size:12.0pt;">:</span></span></span></strong> <strong><em><span style="color:black;"><span style="font-family:b lotus;">این پژوهش به روش مرور نظام‌مند، به بررسی علل و راه‌کارهای کاهش کسورات بیمه‌ای بیمارستان‌های ایران پرداخت.</span></span></em></strong><br> <span style="color:#0000FF;"><strong><span style="font-family:b roya;"><span style="font-size:12.0pt;">روش اجرا:</span></span></strong></span><strong><em><span style="color:black;"><span style="font-family:b lotus;"> پژوهش حاضر به‌صورت نظام‌مند به بررسی مطالعاتی پرداخت که علل یا راه‌کارهای کسورات بیمارستان‌ها را تا سال 1396 بررسی نموده بودند. کلیدواژه‌های کسورات، بیمارستان، صورت‌حساب بیمه و مدارک بیمارستانی با جستجو در پایگاه داده‌ها، موتورهای کاوش و پایگاه مجلات مرتبط مورد بررسی قرار گرفتند. برای هر مقاله فرم استخراج داده تکمیل و در نهایت داده‌ها طبقه‌بندی، خلاصه‌سازی و تحلیل گردید.</span></span></em></strong><br> <strong><span style="color:#0000FF;"><span style="font-family:b roya;"><span style="font-size:12.0pt;">یافته‌ها: </span></span></span><em><span style="color:black;"><span style="font-family:b lotus;">در جستجوی اولیه، 114 مقاله یافت شد. با توجه به معیارها، 21 مقاله تحلیل شدند. علل کسورات در 15 طبقه و 2 حیطه شامل اشکالات مربوط به مستندسازی پرونده همراه با ارائه خدمات و اشکالات مربوط به تهیه، تنظیم و ارسال اسناد دسته‌بندی گردید. راه‌کارهای کاهش کسورات در 9 حیطه اصلی اصلاح برنامه‌ریزی‌ها، آموزش، نظارت، اصلاح فرایند ثبت، امور حاکمیتی، اصلاح فرایند مستندسازی، اصلاح ساختار، رعایت مقررات سازمان بیمه‌گر و اصلاح فرایند ارسال طبقه‌بندی شدند.</span></span></em></strong><br> <span style="color:#0000FF;"><strong><span style="font-family:b roya;"><span style="font-size:12.0pt;">بحث و نتیجه‌گیری:</span></span></strong></span> <strong><em><span style="color:black;"><span style="font-family:b lotus;">اشتباهات محاسباتی، نبود نظارت از سوی بیمارستان و اشکالات مربوط به تاریخ و ساعت اسناد از مهم‌ترین علل کسورات و اصلاح برنامه‌ریزی‌ها ازجمله ایجاد نظام بازپرداخت کارآمد نظری </span></span></em></strong><strong><em><span dir="LTR"><span style="color:black;"><span style="font-family:times new roman,serif;">DRG</span></span></span></em></strong><strong><em><span style="color:black;"><span style="font-family:b lotus;">، آموزش و نظارت مهم‌ترین راه‌کارهای کاهش کسورات بودند. بررسی مقالات به‌لحاظ حرفه‌ی درگیر نشان داد که پزشکان متخصص و پرستاران بیش‌ترین نقش را در ایجاد کسورات دارند. به‌لحاظ نوع خدمات ارائه‌شده، جراحی، خدمات پاراکلینیک و دارو در صدر علل کسورات قرار داشتند. بیمارستان‌های تابعه وزارت بهداشت که با مشکل عدم کفایت درآمدهای بیمارستانی مواجهند و نیز مدیران ستادی برای شناخت کیفیت عملکرد مالی این مراکز می‌توانند از نتایج این مطالعه بهره‌مند شوند</span></span></em></strong><strong><em><span dir="LTR"><span style="color:black;"><span style="font-family:b lotus;">.</span></span></span></em></strong> <div style="text-align: justify;"><span style="color:#0000FF;"><strong>Insurance Deductions of Hospitals in Iran: Systematic Review of Causes and Solutions to Reduce Deductions</strong></span><br> <br> Aidin Aryankhesal¹, Hadi Kalantari¹, Pouran Raeissi¹, Niusha Shahidi Sadeghi^{1, *}<br> <em>¹</em><em>Department of Health Services Management, School of Health Management and Information Sciences, Iran University of Medical Sciences, Tehran, Iran</em><br> <br> <strong><span style="color:#0000FF;">Abstract</span></strong><br> <strong>Background: </strong>This study was conducted via a systematic review of causes and solutions to reduce insurance deductions of hospitals in Iran.<br> <strong>Methods: </strong>This study was conducted via a systematic review. The researchers reviewed studies that investigated the causes or solutions to hospital deductions or both of them up to 2017, based on keywords that included deductible, hospital, bill of insurance, and hospital records through databases, search engines, and related journals. The extracted data form for each article was completed and finally, the data were categorized, summarized, and analyzed.<br> <strong>Results: </strong>In the initial search, 114 articles were found. After reviewing the inclusion and exclusion criteria, 21 articles were analyzed. The main categories of the reasons for deductions were comprised of 15 areas that were classified into the two main areas of documentation flaws during the provision of services and the difficulties with the preparation, adjustment, and sending documents. Analyzing the articles for the reduction of deductions showed that these solutions were classified into 9 main areas of reforming the planning, training, monitoring, reforming the registration process, governance issues, reforming the documentation process, reforming the structure, complying with the rules of the insurance organization, and modifying the process.<br> <strong>Conclusions: </strong>Computational errors, lack of hospital supervision, and errors related to the date and time of the documents constituted the main causes of deductions, and the reform of planning, education, and supervision were among the most important suggested ways to reduce deductions. Reviewing professionally related papers showed that specialist doctors and nurses are professionals who are more involved in making deductions. In terms of the type of offered services, the surgery, paraclinical services, and the drug were at the top of the reasons for deductions.
