fa سنجش عملکرد واحد اورژانس بیمارستان‌های تحت پوشش دانشگاه علوم پزشکی تهران با استفاده از روش کارایی برتر Efficiency Measurement for Emergency department of Tehran University of Medical Science’s Hospitals by using additive super-efficiency method پژوهشی Original <div style="text-align: center;"></div> <div style="text-align: justify;">چکیده مقدمه: رشد هزینه‌های بخش بهداشت و درمان با توجه به کمبود منابع مالی، ضرورت ارزیابی عملکرد واحدهای مختلف را از نظر کارایی توجیه می‌کند. هدف پژوهش حاضر تعیین کارایی واحد اورژانس بیمارستان‌های تحت پوشش دانشگاه علوم پزشکی تهران با استفاده از روش کارایی برتر و مقایسه آن با روش معمول تحلیل پوششی داده‌ها بود. روش کار: مطالعه‌ی حاضر مطالعه‌ای توصیفی- تحلیلی بود که به بررسی عملکرد واحدهای اورژانس 16 بیمارستان دانشگاه علوم پزشکی تهران با استفاده از کارایی برتر و اسلک‌ها با فرض مدل بازدهی ثابت به مقیاس و نهاده محور، طی سال 1394 پرداخته است که به منظور تحلیل داده‌ها از نرم‌افزار  DEA-Solver استفاده گردید. یافته‌ها: بر اساس نتایج حاصل از برآورد کارایی فنی با استفاده از مدل بنکر، چارنز و کوپر مشخص گردید که 5 واحد از نظر فنی کارا شده‌اند (31% از واحدها)، در حالی که به‌دلیل قدرت بالای مدل کارایی برتر در تفکیک واحدهای کارا، تنها 4 واحد کارا شدند به عبارت دیگر فقط 25% از واحدهای اورژانس دارای عملکرد کاملاً کارا بودند. با توجه به اسلک‌های محاسبه شده اکثر واحدها با مازاد نهاده پزشک مواجه نبودند، در حالی که واحدهای اورژانس بیمارستان‌های روزبه و رازی با وجود دارا بودن کارایی برتر از نظر اسلک‌ها با کسری ستاده پذیرش‌های بستری مواجه بودند. نتیجه‌گیری: شناسایی میزان اسلک واحدهای مختلف با استفاده از روش کارایی برتر به مدیران و سیاست‌گذاران در زمینه تصمیم‌گیری برای تخصیص بهینه منابع کمک خواهد نمود تا از این طریق بتوانند از هدر رفت منابع جلوگیری نمایند.  گل‌واژگان: کارایی برتر، اسلک‌ها، تحلیل پوششی داده‌ها، اورژانس</div> <div style="text-align: center;">Efficiency Measurement for Emergency department of Tehran University of Medical Science’s Hospitals by using additive super-efficiency method</div> <div style="text-align: justify;"> Sara Emamgholipour (PhD)1, Marita Mohammadshahi (PhD Student)2*, Behzad Raei (PhD Student)3, Maryam Radinmanesh (PhD Student)4, Ramin Rahimnia (PhD Student)5</div> <div style="text-align: center;">1 Associate Professor, PhD in economics, Department of Health Management and Economics, School of Public Health, Tehran University of Medical Sciences, Tehran, Iran. 2 MS, PhD Student of Health Economics, Department of Health Management and Economics, School of Public Health, Tehran University of Medical Sciences, Tehran, Iran. 3 MS, PhD Student of Health Economics, Department of Health Management and Economics, School of Public Health, Tehran University of Medical Sciences, Tehran, Iran. 4 MS, PhD Student of Health Economics, School of health management and information sciences, Iran University of Medical Sciences, Tehran, Iran. 5 MD, PhD student of Nano medicine, director of financial resources planning office, Vice chancellor of management development resources planning, Tehran University of Medical Sciences, Tehran, Iran.</div> <div style="text-align: center;"> <div style="text-align: justify;">Abstract  </div> <div style="text-align: justify;">Introduction: The growth of health care costs due to the lack of financial resources justifies the need to evaluate the performance of various units via efficiency approaches. The aim of the present study was to determine the efficacy of the emergency unit of Tehran University of Medical Sciences’s hospitals by using an additive super efficient method and then compare it with the usual method of data envelopment analysis. Methods: This was an analytical-descriptive study, which examined the performance of 16 hospital emergency departments of Tehran University of Medical Sciences. We calculated super efficiency and slacks of inputs and outputs for each hospital by assuming constant returns to scale and input-oriented model by using data for the year 2014-2015 via the DEA-Solver software. Results: The results of Malmquist model showed that 8 departments had an efficiency of about 50%, although, with the high power of super efficiency approach, only 5 departments have efficiency. In other words about 33% of the departments have perfect performance. According to the calculated slacks, none of the departments had excess inputs, however, Razi and Rozbeh emergency departments had lack of outputs for inpatient admission. Conclusion: Identifying the amount of slack in different units will help managers and policymakers to decide on the optimal allocation of resources by using a superior efficient method to avoid waste of resources. Since the managers and policy makers need to assess the exact performance of hospitals in terms of strengths and weaknesses for inputs and outputs, using the super efficiency model, relative to Malmquist, will get more exact results for decision making.
