fa بررسی موانع و تسهیل‌کننده های ارائه خدمات در پایگاه‌های سلامت مشارکتی و دولتی تحت پوشش دانشگاه علوم پزشکی تهران: یک مطالعه کیفی عمومى General پژوهشی Original <div style="text-align: center;"></div> <div style="text-align: justify;">&nbsp;<span style="color:#0000FF;"><strong><span style="font-family:b roya;"><span style="font-size:12.0pt;">مقدمه:</span></span></strong></span><strong><span style="color:#0000FF;"><span style="font-family:b lotus;"> </span></span><span style="color:black;"><span style="font-family:b lotus;">مطالعه کیفی حاضر با هدف شناسایی موانع و تسهیل‌کننده های ارائه خدمت در پایگاه‌های سلامت تحت پوشش دانشگاه علوم پزشکی تهران به‌انجام رسید.</span></span></strong><br> <span style="color:#0000FF;"><strong><span style="font-family:b roya;"><span style="font-size:12.0pt;">روش‌مطالعه:</span></span></strong></span><strong><span style="color:black;"><span style="font-family:b lotus;"> برای انجام این مطالعه با کارشناسان مصاحبه به عمل آمد، پیاده‌سازی انجام‌شد، و نمایه‌سازی </span></span>ر</strong>وی تمامی مصاحبه ها صورت‌گرفت. تمامی افراد و موضوعات کدگذاری‌شده و متعاقبا، جدول مربوط به نمودارها تکمیل گردید. سپس بر اساس فرم ساختار مفهومی، نمایهِ‌سازی انجام، و داده ها تجزیه‌و‌تحلیل شد.<br> <span style="color:#0000FF;"><strong><span style="font-family:b roya;"><span style="font-size:12.0pt;">یافته</span></span></strong><strong><span dir="LTR"><span style="font-size:12.0pt;">‌</span></span></strong><strong><span style="font-family:b roya;"><span style="font-size:12.0pt;">ها: </span></span></strong></span><strong><span style="color:black;"><span style="font-family:b lotus;">دیدگاه شرکت‌کندگان نسبت به تجربه مثبت بود. مهم‌ترین تسهیل‌کننده‌ها، استقبال بخش خصوصی، اشتغال‌زایی، دستورالعمل‌های شفاف،اعتبارات مشخص جهت پرداخت، و تجربیات سایر کشورها بود. عمده‌ترین موانع، نامناسب بودن فضای فیزیکی، کمبود نیروی انسانی، عدم استقرار پزشک، فرسوده بودن تجهیزات، تاخیر در پرداخت به موسسین، حقوق ناکافی پرسنل، نظارت‌های سختگیرانه ناظران ستادی، توجه به کمیت به جای کیفیت خدمات، قوانین دست و پا گیر بیمه، و مالیات بود.</span></span></strong><br> <span style="color:#0000FF;"><strong><span style="font-family:b roya;"><span style="font-size:12.0pt;">نتیجه</span></span></strong><strong><span style="font-family:b lotus;"><span style="font-size:12.0pt;">‌</span></span></strong><strong><span style="font-family:b roya;"><span style="font-size:12.0pt;">گیری:</span></span></strong> </span><strong><span style="color:black;"><span style="font-family:b lotus;">سیاست کوچک کردن دولت، واگذاری خدمات به بخش خصوصی، و نظارت سیستم دولتی سبب شکل‌گیری نگرش مثبت در مدیران نسبت به پایگاه‌های مشارکتی شده‌، و از سویی دیگر عدم کارایی پرسنل دولتی به راه‌اندازی موفقیت‌آمیز پایگاه‌های مشارکتی انجامیده‌است. محدودیت‌های مالی و اداری در خرید، ساخت، و تعمیر ساختمان‌های دولتی و نیز کاهش ظرفیت استخدامی از دلایل واگذاری پایگاه‌های مشارکتی است. همچنین، نیروهای شاغل در پایگاه‌ها از عدم امنیت شغلی و حقوق پایین شکایت داشتند. </span></span></strong></div> <div></div> <strong><span style="color:#0000FF;">The Study of Barriers and Facilitators of Providing Services in participatory and Public Health Facilities Covered by Tehran University of Medical Sciences: A Qualitative Study</span></strong><br> <br> <strong>Mohammad Reza Ghayoom Zade ¹, Arash Rashidian ², Farid Abolhassani ³, Elham Rashidian ^1*, Mohammad Reza Salmani ⁴, Sepideh Samimi⁵</strong><br> <em>¹</em><em> South Tehran Health Network, Deputy for Health, Tehran University of Medical Sciences, Tehran, Iran</em><br> <em>²</em><em> Department of Health Management and Economics, School of Public Health,<br> Tehran University of Medical Sciences, Tehran, Iran</em><br> <em>³</em><em> National Institute of Health Research, Tehran University of Medical Sciences, Tehran, Iran</em><br> <em>⁴</em><em> Rey Health Network, Deputy for Health, Tehran University of Medical Sciences, Tehran, Iran</em><br> <em>⁵</em><em> Pardis International School of Medicine, Tehran University of Medical Sciences, Tehran, Iran</em><br> &nbsp; <div style="text-align: justify;"><span style="color:#0000FF;"><strong>Abstract</strong></span></div> <div><strong>Background: </strong>This study is a qualitative study that was conducted at Tehran University of Medical Sciences with the aim of identifying obstacles and facilitators of providing services at participatory sites covered by Tehran University of Medical Sciences.<br> <strong>Methods: </strong>Experts were interviewed, the implementation was carried out, and the indexing process was performed on all interviews. All coding subjects and topics as well as the charts table were completed. Then, based on the conceptual structure form, indexing and data analysis were done.<br> <strong>Results: </strong>The process was positive toward the experience. The most important facilitators were the private sector, job creation, transparent guidelines, specific credits for payments, and experiences from other countries. Some of the main obstacles were inadequate physical space, shortage of manpower, lack of physician placement, weariness of equipment, delayed payments to founders, insufficient salaries for staff, strict supervision of staff oversight and attention to quantity rather than service quality, insurance and taxes.<br> <strong>Conclusions: </strong>The policy of small government, the privatization of services to the private sector, and supervision by the state system has led to a positive attitude of managers towards participatory bases. In addition, the ineffectiveness of government personnel from other countries has led to the creation of participatory bases successfully. The financial and administrative constraints in purchasing, constructing, and repairing state civil-engineering units and reducing employment capacity are also due to the transfer of participatory bases.
