fa تامین منابع فیزیکی و الزامات اجرای برنامه پزشک خانواده شهری در ایران: از تئوری تا عمل عمومى General پژوهشی Original <div style="text-align: center;"></div>&nbsp;<strong style="font-size: 14px; text-align: justify;"><span style="color: rgb(0, 0, 205);"><span style="font-family: b roya;">مقدمه</span></span><span style="color: black;"><span style="font-family: b roya;">:</span></span></strong><span style="font-size: 14px; text-align: justify;"> </span><strong style="font-size: 14px; text-align: justify;"><span style="color: black;"><span style="font-family: b lotus;">برنامه پزشک خانواده شهری به‌عنوان یک برنامه جدید تندرستی، مستلزم تامین منابع و امکانات مورد نیاز می‌باشد. این مطالعه به منظور بررسی وضعیت منابع فیزیکی و الزامات مرتبط در برنامه پزشک خانواده شهری استان‌های فارس و مازندران انجام گرفت.</span></span></strong> <div style="text-align: justify;"><span style="font-size:14px;"><span style="color: rgb(0, 0, 205);"><strong><span style="font-family: b roya;">روش</span></strong><strong><span dir="LTR">‌</span></strong></span><strong><span style="color: rgb(0, 0, 205);"><span style="font-family: b roya;">کار</span></span><span style="color: black;"><span style="font-family: b roya;">:</span></span></strong><strong><span style="color: black;"><span style="font-family: b lotus;"> مطالعه از نوع مقطعی بوده و در زمستان 1395 انجام گرفت. با توجه به برآورد حجم نمونه، 134 مرکز و پایگاه سلامت شهری مجری برنامه پزشک خانواده در استان فارس و 96 مرکز و پایگاه سلامت شهری در استان مازندران به‌‌روش تصادفی منظم به‌عنوان نمونه پژوهش انتخاب شدند. ابزار جمع‌آوری داده‌ها، پرسشنامه محقق ساخته بود که روایی و پایایی آن مورد تائید قرار گرفت. داده‌ها با آزمون‌های‌‌ کروسکال-والیس و مجذور کای تحلیل شد.</span></span></strong><br> <span style="color: rgb(0, 0, 205);"><strong><span style="font-family: b roya;">یافته</span></strong><strong><span dir="LTR">‌</span></strong><strong><span style="font-family: b roya;">ها: </span></strong></span><strong><span style="color: black;"><span style="font-family: b lotus;">از 233 مرکز و پایگاه مطالعه شده، در 105 واحد (3/45%) بیش از یک پزشک مستقر بودند. در خصوص وضعیت سازه‌ای، فضای فیزیکی 183 واحد (9/79%) و نظافت عمومی 178 واحد (1/78%) مطلوب بود. نصب مشخصات ارایه‌دهندگان خدمات در برنامه پزشک خانواده شهری از 1/42% تا بیش از 70% متغیر بود. همچنین، 104 واحد (8/45%) دارای الکتروکاردیوگرام (</span></span></strong><strong><span dir="LTR"><span style="color: black;"><span style="font-family: times new roman,serif;">EKG</span></span></span></strong><strong><span style="color: black;"><span style="font-family: b lotus;">) سالم، 195 واحد (5/85%) دارای آفتالموسکوب سالم، 227 واحد (4/97%) دارای لپ‌تاب یا کامپیوتر سالم، 178 واحد (7/77%) دارای تخت ژنیکولوژی سالم و 125 واحد (8/58%) دارای اتوکلاو سالم بودند. در خصوص تامین منابع فیزیکی و رعایت الزامات در بسیاری از موارد میان استان‌های مجری تفاوت معنی‌داری وجود داشت (05/0></span></span></strong><strong><span dir="LTR"><span style="color: black;"><span style="font-family: times new roman,serif;">P</span></span></span></strong><strong><span style="color: black;"><span style="font-family: b lotus;"> ).</span></span></strong><br> <span style="color: rgb(0, 0, 205);"><strong><span style="font-family: b roya;">نتیجه</span></strong><strong><span style="font-family: b lotus;">‌</span></strong><strong><span style="font-family: b roya;">گیری:</span></strong></span> <strong><span style="color: black;"><span style="font-family: b lotus;">نتایج نشان داد بعضی از امکانات و الزامات در تعدادی از واحدهای مجری تامین نشده که برای تامین آن باید مداخلات متناسب به‌عمل آید</span></span></strong><strong><span dir="LTR"><span style="color: black;"><span style="font-family: b lotus;">.</span></span></span></strong></span></div> <div style="text-align: justify;"><strong><span style="color:#0000CD;">Providing Physical Resources and Requirements for the Implementation of the Family Physician Program in Iran: From Theory to Practice</span></strong><br> <br> Mohammad Javad Kabir ¹, Hasan Ashrafian Amiri ², Seyed Mozaffar Rabiee ³, Reza Momtahen ⁴, Rasoul Zafarmand ⁵, Seiyed Davoud Nasrollahpour Shirvani ^{2 *}<br> <br> <em>¹</em><em>Health Management and Social Determinants Research Center, Golestan University of Medical Sciences, Gorgan, Iran</em><br> <em>²</em><em>Social Determinants of Health Research Center, Health Research Institute, Babol University of Medical Science, Babol, Iran</em><br> <em>³</em><em>Cancer Research Center, Health Research Institute, Babol University of Medical Sciences, Babol, Iran</em><br> <em>⁴</em><em>Directorate of Health Insurance of Fars Province, Shiraz, Iran</em><br> <em>⁵</em><em>Directorate of Health Insurance of Mazandaran Province, Sari, Iran</em><br> <br> <strong><span style="color:#0000CD;">Abstract</span></strong><br> <br> <strong>Background: </strong>The urban physician program as a new health program requires the provision of resources and facilities. This study aimed to investigate the status of physical resources and related requirements in the family physician program in Fars and Mazandaran provinces.<br> <strong>Methods: </strong>This cross-sectional study was carried out in winter 2016. According to the sample size estimation, 98 clinics and centers, in Mazandaran province, and 135 clinics and centers, in Fars province, were selected by a systematic random sampling method. The data collection tool was a researcher-made questionnaire; its validity and reliability were confirmed. Data were analyzed using Kruskal-Wallis and Chi-square tests.</div> <div style="text-align: justify;"><strong>Results: </strong>Out of 233 studied units, 105 (45.3%) had more than one physician. The structural status of the physical space in 183 units (79.9%) and the general cleaning of 178 units (78.1%) were desirable. The specification of service providers in the urban family physician program was completed from 42.1% to over 70%. One hundred and four units (45.8%) had an intact EKG, 195 units (85.5%) had an intact Ophthalmoscope, 227 units (97.4%) had an intact laptop or computer, 178 units (77.7%) had an intact gynecology bed, and 125 units (58.8%) had working autoclaves.
