fa استانداردهای پیشنهادی جهت ارزیابی تولیت در نظام ملی سلامت دهان و دندان: بخش اول- ارزیابی وضع مقررات پژوهشی Original <p align="justify"><strong>  مقدمه:</strong> هدف از این مطالعه دو مرحله‌ای، طراحی مجموعه استانداردهایی برای ارزیابی تولیت نظام سلامت دهان و دندان؛ و ارزیابی وضعیت کنونی کشور ایران بر اساس آن‌ها می‌باشد. </p><p align="justify"> <strong> روش کار:</strong> بر اساس روش تغییر یافته رند ( modified RAND-UCLA ) با مرور جامع پایگاه‌های داده‌ای الکترونیک و سایت رسمی تعدادی از سازمان‌های بین‌المللی، راهکارهای کارای سیاست‌گذاری در حیطه تولیت، استخراج شدند و بر اساس شش زیرحیطه تولید و انتشار خردورزی، تعیین جهت‌گیری راهبردی، فراهم‌سازی زیرساخت‌های سازمانی متناسب با اهداف راهبردی، وضع مقررات، تأثیرگذاری بر سایر حوزه‌ها از طریق رهبری بین‌بخشی و اطمینان از پاسخگویی طبقه‌بندی شدند. میزان اهمیت و ارتباط استانداردهای تدوین شده با استفاده از آرای صاحب‌نظران سنجیده شد. در مرحله دوم، با استفاده از لیست نهایی استانداردها و با مصاحبه با صاحب‌نظران حوزه سلامت دهان و مرور اسناد، تولیت نظام سلامت دهان (دراین مقاله وضع مقررات) مورد ارزیابی نقادانه قرار گرفت. </p><p align="justify">  <strong>یافته‌ها:</strong> از مجموع 38 استاندارد نهایی، 18 استاندارد مربوط به وضع مقررات بود. اکثر استانداردهای نامناسب مربوط به وضع مقررات برای بیمه‌های دندان‌پزشکی بودند. مقررات مربوط به اطمینان از هزینه اثربخش وایمن بودن مواد و تکنیک‌های به کار رفته در دندان‌پزشکی و کیفیت خدمات ارایه شده به بیماران، قوانین مربوط به مبادلات پایه بازار سلامت در دندان‌پزشکی، حق مالکیت معنوی و فلوریداسیون آب ناقص بودند و یا به طورمطلوب پیاده نمی‌شدند. نحوه فعلی اجرایی شدن جریمه‌ها و تنبیهات در برخورد با مصادیق کارکرد نامناسب ارایه‌دهندگان و قوانین مربوط به تربیت و توزیع مناسب ارایه‌دهندگان خدمات دندان‌پزشکی ناکامل بود. قوانین مربوط به برچسب‌گذاری بر کالاها و پوشش نیازهای مناطق محروم به صورت کامل یا نسبتاً کامل موجود بود. </p><p align="justify">  <strong>نتیجه‌گیری:</strong> ا گرچه در حال حاضر وزارت بهداشت برای بیشتر حیطه‌های تنظیمی، که معمولاً ناشی از مصادیق شکست بازار سلامت هستند، دارای مجموعه‌ای از قوانین و مقررات می‌باشد، مشکل اصلی از دید صاحب‌نظران و بر اساس شواهد، ضعف در تقویت، اجرایی شدن و پایش آن‌ها می‌باشد. </p><p align="justify">  </p> <p> <strong> </strong></p><p> <strong> </strong> <strong> </strong></p><p align="center">  Tahani B¹ (DDS, PhD), Yazdani Sh² (MD, MSc.), Khoshnevisan MH³ <a name="_ftnref1"><u>*</u> </a>( PhD, DMD, DPHDent, DrPH ) </p><p align="center">  </p><p align="center"> ¹<i>Torabinejad Dental Research Center, Department of Oral Public Health, School of Dentistry, </i></p><p align="center"> <i> Isfahan University of Medical Sciences, Isfahan, Iran </i></p><p align="center"> ²<i>School of Medical Sciences Education, Shahid Beheshti University of Medical Sciences, Tehran, Iran </i></p><p align="center"> ³<i>Preventive Dentistry Research Centre. Standards were categorized according to the six sub-functions of stewardship including generating and disseminating intelligence defining strategic policy direction ensuring alignment of policy objectives and organizational structure, regulation exerting influence over all related sectors via inter-sectoral leadership and ensuring accountability. Then the level of relevancy and importance of standards were assessed using the experts’ opinions. In phase two, using the finalized list of standards, semi-structured interviews with key stakeholders in oral health system at different levels of policymaking were conducted and the stewardship of oral health system was critically evaluated. The results of regulation sub-function are presented in this study. </p><p> <strong> Results: </strong>From 38 finalized standards appropriate for evaluating the stewardship of oral health system according to the opinion of experts, 18 were related to regulation. Most of the standards rejected were included in dental health insurance regulation. Evaluation of this sub-function determined that currently there are deficiencies in meeting regulations concerned with evaluating the safety and cost effectiveness of dental materials, and quality of provided dental care to patients. Also it was shown that implementation of penalties and sanctions commensurate with instances of malpractice at individual and institutions were not completed attained. Regulations such as water fluoridation, property rights and regulations to ensure the basic conditions of market exchanges were either absent or only partially implemented. There were also some weaknesses in regulations concerning the supply of dentists, especially their distribution. The fields that currently had some regulations, were those in relating to labeling on products used in dentistry and covering the needs of disadvantaged populations . </p><p> <strong> Conclusion: </strong>While the Ministry of Health and Medical Education has achieved some stewardship measures in the field of regulation, there are some problems in enforcing the regulatory framework especially in the private sector. </p><p>  </p><p> <strong> Key words: </strong>oral health system, evaluation, stewardship, legislation </p><p> <strong> </strong></p><p>  </p><br clear="all" ><p> <strong> Please cite this article as follows: </strong></p><p>  Tahani B, Yazdani Sh, Khoshnevisan MH.
