Abstract

Introduction: Setting tariffs is one of the most important tools of policy makers at Health system in every country for applying stewardship which is effective on justice, effectiveness, quality and accountability of health services, and also could influence the accessibility and the rate of using health care services. The changing of medical tariffs not only could affect the income of medical and hospital service providers, but also  could affect the costs of the funders of medical and hospital services. This study was conducted to investigate the concept of real tariff from insurance organizations managers’ point of view.

Methods: This qualitative study was conducted using in-depth interview with 19 informed managers, or relevant managers of insurance organizations, social security and armed forces. Data was analysed thematic analysis method.

Results: In general four main challenges were reported by participants as the challenges of tariffs including: methods, structure and principles of tariff setting; economic aspects, dissatisfaction of professionals involved with tariff setting, and lack of supervision on non-tariff expressed, and implementation challenges.

Conclusion: The identified challenges should be taken into account to modify the tariff setting, and to identify problems. The practical principles should be considered to determine tariffs as well as the scientific and logical principles.

Abstract

Challenges of the Rural Insurance of Family Physician Program in Khorasan Razavi Province: A Qualitative Study

Abstract
Background: Rural insurance has been designed and implemented in our country to develop social justice. In the health system, identifying its challenges to improve and implement reforms is essential. The present study aimed to assess the quality of rural insurance challenges in Khorasan Razavi Province.
Methods: This study was conducted with a qualitative approach and a content analysis method in 2015 in Mashhad. Eighteen managers, policy makers, family physicians, and insurers who were selected by targeted sampling until data saturation were reached. After conducting the interviews, data were analyzed through conventional content analysis approach using the MAXQDA software version 10.
Results: Data analysis consists of two main themes including: Management challenges of the rural insurance system (Inefficient financial management of credits, non-comprehensive rules, inadequate implementation of guidelines, and deficiencies in the health information management system), and cultural challenges (lack of referral system observance and inappropriate cultural context). Each category contained several subcategories. 
Conclusions: Inefficient financial management of credits, lack of comprehensive guidelines, lack of information management and inappropriate cultural context of providers are essential challenges of rural insurance challenges in Mashhad. Since the rural insurance system plays a key role in the success of family physician policy and the development of social justice, it is necessary to use the results of this study to solve the problems and challenges of rural insurance.

Abstract

Introduction: Organizational leadership is ‘the science and art of influencing employees, and mobilizing them to achieve organizational goals’. The purpose of this research was to identify the leadership challenges of female managers in healthcare organizations.
Methods: This interpretive phenomenological research was conducted through in-depth semi-structured face-to-face interviews with 27 female managers in healthcare organizations in Tehran, Iran. The research sample was selected by purposeful and snowball methods. Thematic analysis method and MAXQDA2018 were used to analyze the data.
Results: Overall, 20 challenges were identified in the leadership of female managers of healthcare organizations, which were classified into four groups of structural, process, human, and cultural challenges. Personal and personality characteristics of female managers such as kindness, compassion, and flexibility, along with little management experience, insufficient authority, inconsistency with superior and peer managers, lack of resources, high workload, low motivation, knowledge and skills of subordinates, informal groups, organizational masculinity, gender misconceptions, and organizational discrimination were among the most important leadership challenges of female managers of healthcare organizations. Some of the solutions to solve these challenges include strengthening intelligence, knowledge, and management skills, adhering to moral values, managing gender characteristics, delegating authority, creating resources, increasing risk-taking, improving communication, effective conflict management, developing communication networks, meditation, coaching, correcting gender stereotypes and removing gender discrimination.
Conclusion: Female managers face several challenges in leading their employees in healthcare organizations that need to be identified and resolved. A model was developed to strengthen the leadership of female managers of healthcare organizations, which will lead to an increase in their productivity.
